The people leave first.
On any matter that runs long enough, turnover does its quiet work. By the time a question is properly asked, most of those who held the answer have gone, and what they knew went with them: why a decision was taken, what was agreed in the corridor, who approved the change.

The record collapses second.
People under pressure do not file. The shared drive sits empty; the document system is used by the diligent few and ignored by everyone else. A record that was never centralised cannot be searched, weighed, or produced at the moment it matters.
What remains, in almost every case,
is the correspondence.
